FY2027 ICD-10 PHYSICAL THERAPY UPDATES
FY 2027 ICD-10 UPDATE: The straightforward operational rules to effectively implement the changes affecting Physical Therapy on October 1st, 2026.
Effective October 1st, 2026
The FY 2027 ICD-10-CM update is now in effect. It includes 190 new diagnosis codes, 30 deleted codes, and four revised descriptions. For physical therapy practices, several of those changes may affect diagnosis selection, documentation, claim edits, and episodes of care that begin on the October 1st effective date.

Sound Medical Billing Solutions has reviewed the FY 2027 ICD-10-CM updates taking effect October 1, 2026. This blog provides practical guidance to help physical therapy practices identify, understand, and immediately apply the changes that may impact documentation, coding, and billing.
✋ Five Changes Effecting Physical Therapy Practices
Topic | FY 2027 coding | Practice note |
Plantar fasciitis | M67.A01 right, M67.A02 left, M67.A09 unspecified | M72.2 is no longer used for plantar fasciitis. Verify laterality. |
Plantar fascial fibromatosis | M72.21 right, M72.22 left, M72.20 unspecified | Clarify the diagnosis when the record says only plantar fascia pain. |
Gait and balance | R26.2 and R26.81 may be reported together | Use both only when each condition is present, documented, and relevant. |
Osteomyelitis | More specific site and laterality codes (see the M86 family) | Match the diagnosed anatomical site and side in the medical record. |
Sternoclavicular sprain | S23.420A D and S are deleted; review S43.6 | Confirm laterality and phase of care before replacing a code. |

📝 Coding and Documentation Guidance
📌Plantar fasciitis has a new code family
Plantar fasciitis now has laterality-specific codes. Report M67.A01 for the right foot, M67.A02 for the left foot, and M67.A09 when the foot is unspecified. M72.2 is no longer used for plantar fasciitis.
This is a likely source of denials starting on October 1st. If M72.2 remains in an EMR favorite, evaluation template, or billing rule, staff may continue selecting a deleted code. Clinical documentation should identify the affected foot so the unspecified code is reserved for records that truly lack laterality.
📌Plantar fascial fibromatosis requires laterality
Plantar fascial fibromatosis now maps to M72.21 for the right foot, M72.22 for the left foot, and M72.20 when the foot is unspecified. This diagnosis is distinct from plantar fasciitis.
A referral that lists only plantar fascia pain may not establish which condition is present. Clarify the diagnosis when needed rather than assuming that the symptom represents plantar fasciitis or fibromatosis.
📌Gait and balance codes may be reported together
R26.2 for difficulty in walking and R26.81 for unsteadiness on feet may be reported together when both conditions are present, separately documented, and relevant to the plan of care. The FY 2027 tabular instruction changes from Excludes1 to Excludes2.
The change permits the combination; it does not require it. Do not add both codes automatically. The evaluation and treatment record should show the distinct gait and balance problems that support each diagnosis.
📌Osteomyelitis coding becomes more specific
Several former osteomyelitis codes are replaced with codes that provide greater detail about the anatomical site and laterality. This is particularly relevant to wound care, limb preservation, postoperative therapy, and other medically complex rehabilitation services. M86.8X1 - M86.8X8 are removed. See the M86 family of replacements of body part and laterality.
Documentation should match the diagnosed site and side in the medical record. Do not infer the type or location of osteomyelitis from the wound location alone. When the record is unclear or conflicting, coordinate with the diagnosing provider before selecting a replacement code.
📌Sternoclavicular sprain codes move to S43.6
S23.420A, S23.420D, and S23.420S are deleted. Applicable sternoclavicular joint sprain coding is found in the S43.6 family, which distinguishes laterality and uses a seventh character for the phase of care.
Review the medical record before replacing an S23.420 code. The correct selection depends on the affected side and whether the encounter involves active treatment, subsequent care, or a sequela. A blind one-to-one replacement can produce an inaccurate claim.
✅ Your October 1st Readiness Checklist
Practices should focus on active accounts, saved diagnosis lists, and claim edits that may still contain deleted or outdated codes.

1. Review full list of ICD-10 FY 2027 edits: https://www.icd10data.com/
2. Review active patient accounts Identify episodes that use affected or deleted codes, especially services continuing across October 1.
3. Update your EMR Remove outdated codes from favorites, evaluation templates, smart phrases, order sets, and internal reference sheets.
4. Verify laterality Confirm that the record identifies the right or left side whenever the active code requires it.
5. Check gait and balance edits Make sure billing rules no longer reject R26.2 and R26.81 solely because of the former Excludes1 instruction.
6. Contact SMBS Contact SMBS to help clarify and update identified accounts.
👉 SMBS Final Thoughts
The most important operational rule is straightforward: do not submit deleted ICD-10-CM codes for services provided on or after October 1, 2026. Your documentation must support the most specific active code, including the diagnosis, anatomical site, laterality, encounter phase, and functional problem when those details apply. A focused review now can prevent avoidable claim edits, and rejected and unpayable insurance claim submissions.
